New Patient Health Checks (for children up to 16 years)
All newly registered patients are required to attend the Surgery for a health check within one month of registering, although medical treatment is available from the date of registration.
The Receptionist will make an appointment for you to see the practice nurse or healthcare assistant for this health check.
This form is for children under 16, the following form should be completed by a parent or guardian.
